  • Most accurate evaluation results are consistently achieved when all initial questionnaires are VERY THOROUGHLY completed by all involved family members.
  • Young (under 30) adults are strongly advised to seek assistance by a parent with Intake Questionnaires.  The sections addressing extended Family History * and Childhood problems are very important and almost always unknown or incompletely remembered by young adults!
  • Married adults of all ages are always far more accurately evaluated and effectively treated when a spouse is present during the assessment.
  • Grandparents*, aunts, uncles older siblings, and in some cases cousins are often very helpful when included in the initial assessment.  This is especially encouraged when we are evaluating children and adolescents.
  • Individual adolescents and adults are always afforded an opportunity for a private, confidential discussion with Dr. Clark when they wish to discuss certain personal issues which they choose to withhold from family members.  An unrelated staff member must always be present in any such meeting as a chaperon.

*Grandparents often know very important aspects of the Family History which may have never been revealed to adult children.
